Please complete both the Intake Form and Privacy / Consent Forms to request an intake appointment.  We will contact you to schedule your appointment once the forms are submitted. You must complete both forms.

If you inquiring about ADHD or continuing stimulant medication for ADHD, please review and submit our Stimulant Policy.

Initial appointments cannot be scheduled online.

Our office accepts patients who have Blue Cross / Blue Shield insurance coverage. We also accept self-pay patients who are responsible for payment at the time services are rendered. We do not accept any other insurance plans other than Blue Cross / Blue Shield.

Once we receive your completed Intake Form, and signed Privacy Policy & Consent Form, we will contact you to schedule your first appointment.
