Substance Abuse Evaluations & Risk of Recidivism

Middlesex Psychiatry, PLLC performs substance abuse evaluations for individuals who are appealing their license suspensions before the Massachusetts Registry of Motor Vehicles and Division of Insurance Board of Appeal.

An experienced Psychiatric Nurse Practitioner will provide a report which lists the risk of recidivism, which is also known as the risk of relapse.

Risk of recidivism is determined by the interview, a review of documents, and the results of rating scales such as the Alcohol Use Disorders Identification Test. When assessing recidivism risk, two key factors are a substantial period of abstinence / sobriety and some support system in place to prevent a relapse.

This will be an unbiased evaluation which means that it may or may not be favorable to your license reinstatement case.

Substance abuse evaluations are conducted remotely via telehealth. You must have a reliable wifi / internet connection which supports video conferencing.

Please email a copy of any relevant documents such as police reports, the immediate threat complaint (if applicable) and/or your driving record to jen@middlesexpsych.com.

Please complete our Substance Abuse Evaluation Questionnaire to request an appointment.

Payment is required in advance to confirm your appointment and it is non-refundable. You will receive an invoice and you can pay online. Since these evaluations are performed for legal purposes and not for medical treatment, they are not covered by insurance.
